Transformation Teams

The goals of the Transformation Teams are to: assess feasibility of recommendations, identify considerations for implementation of new policies/practices, elevate county and program participant needs, and help to translate policy and training into county culture and practice.

Transformation Teams are organized by county size—small, medium and large. These cohorts meet individually on the first Tuesday of each month to provide input on specific Transformation topics;  they then all come together on the third Tuesday of each month to align their considerations into consolidated input to forward to the Steering Committee
